- Relevant resolution of the General Body meeting for the purpose.
- An affidavit in original (sworn before a magistrate) in non judicial stamp paper by the President or the Secretary (as the case may be), explaining the circumstances of the loss of the original registration certificate of the society (as the case may be).
- Police report regarding the loss of original Registration Certificate.
- Particulars of other relevant documents (other than the registration certificate), like resolution of the general body meeting or AGM, Annual Audit Report, Activity Report of the society.
- Scanned copy of registration certificate.

TIMELINE
- 8 (eight) working days from the date of submission of application, complete in all respects along with documents as per checklist.
